UnitedHealth Group had a mixed 2025: revenue jumped 12% to $447.6 billion, but profitability took a hit as the company absorbed major one-time charges tied to restructuring, a cyberattack, and portfolio exits. GAAP earnings fell to $13.23 per share, with fourth-quarter results nearly wiped out by these costs, while adjusted earnings were stronger at $16.35. Growth at UnitedHealthcare and Optum Rx was offset by sharp margin pressure from Medicare funding cuts, IRA impacts, and rising medical costs, with Optum Health swinging to a loss after a very strong prior year.





Medical costs climbed meaningfully, pushing the medical care ratio higher, though operating expenses stayed tightly controlled. Looking to 2026, UNH is intentionally pulling back from lower-margin businesses, guiding to slightly lower revenue but improved earnings and margins, framing the year as a reset aimed at more stable and disciplined growth rather than sheer scale.
