UnitedHealth Group (UHG) reported strong Q1 2025 revenue growth but earnings slightly below expectations due to higher Medicare-related care costs and changes in Optum Health member profiles. Total revenues reached $109.6 billion, up $9.8 billion year-over-year. Net earnings were $6.85 per share, with adjusted earnings of $7.20 per share. The consolidated operating margin was 8.3%, the medical care ratio was 84.8%, and the operating cost ratio improved to 12.4%. Cash flows from operations were $5.5 billion, and the company returned about $5 billion to shareholders through dividends and share repurchases.

For the full year 2025, UHG revised its outlook, projecting net earnings of $24.65 to $25.15 per share and adjusted earnings of $26.00 to $26.50 per share. The revision reflects higher-than-expected Medicare Advantage activity and changes in Optum Health member profiles, which management believes are addressable and expects to resolve over the course of the year.



By segment, UnitedHealthcare reported Q1 revenues of $84.6 billion and operating earnings of $5.2 billion, with total consumers served increasing by 780,000. Optum posted Q1 revenues of $63.9 billion and operating earnings of $3.9 billion. Within Optum, Optum Health revenues were $25.3 billion, impacted by member profile changes, Optum Rx revenues were $35.1 billion with 408 million adjusted scripts, and Optum Insight revenues reached $4.6 billion with a backlog of $32.9 billion, launching new AI-powered claims processing tools.
